Official Leadership Program Registration Form

 – rThe Official Leadership Program trains teens (13-18 year olds) to become paid umpires and leaders on and off the field. To register, select the location in the drop-down menu and fill out the liability waiver and photo consent form. Once submitted, you will receive a confirmation email from UMPS CARE with more information on your six-week program or weekend clinic. This includes meeting time, location, goals, and more. *Limited space is available for the six-week program so please do not sign up unless you can commit. Questions? Why should you join the Official Leadership Program?

Des Moines, 6-week program at Hoover High School [FULL]
February 28 – April 13
Wednesdays, 6:00-8:00 PM 
2/28, 3/6, 3/20, 3/27, 4/3, 4/10
*No Class March 10
*Optional hands-on clinics 3/2, 3/23, 3/30

In exchange for consideration of participation in this event I grant to UMPS CARE Charities, and its partner organizations (including but not limited to Major League Baseball as well as all Major and Minor League Teams), along with its representatives and employees of each the right to use audio or video recordings, photographs or filmed footage of me and/or my child
as well as any likeness, image or sobriquet in connection with the above-identified subject. I authorize UMPS CARE Charities, its assigns and transferees to copyright, use and publish the audio or video recordings, photographs or filmed footage of me and/or my child as well as any likeness, image or sobriquet in print, video and/or electronically. I also agree that the use and publication of such images, photographs/footage or sobriquet of me and/or my child may be done without my name and said use is for any lawful purpose, including but not limited to publicity, illustration, advertising, and Web content.

I further consideration for participation in this event, I release, acquit and forever discharge UMPS CARE Charities, and its partner organizations (including but not limited to Major League Baseball as well as all Major and Minor League Teams), along with its representatives and employees of each from any and all liabilities or claimed damages for injuries (mental or physical) to myself and or my child while participating in this event.
